This form is intended to provide current CSDC Vendor Members with access to CSDC's new online vendor directory for charter schools.

Please note that we will use the information provided to create a complimentary expanded directory listing (see example) for the duration of your present membership term. As your renewal date approaches, you will either need to select the appropriate upgrades or become a premium vendor member to maintain this expanded listing.

If you are interested in placing a banner advertisement on the directory, please contact CSDC to pay for the banner advertisement cost, which will be prorated according to the duration of your current membership term.

Please provide a brief (<250 characters) company description that will appear on Vendor Directory index pages. This description should pique visitors' interest, encouraging them to click-through to your company's page. See example.

Upload your company's logo in JPG format. The display size for a vendor directory logo is 150px by 150px.
